Making Consumer/Patient Engagement Meaningful

With the change in the sickness patterns of populations around the world and especially in the United States, we are living longer and therefore having to learn to live with more and more chronic conditions such as asthma and diabetes.  And in order to manage healthcare expenditures, payers (and increasingly providers) all have an incentive to keep these chronic populations as healthy as possible and out of receiving care once these conditions get out of hand.

But how do we get patients (e.g. consumers) to help themselves in order to help us (payers/providers)?  It turns into a bit of a chasing game.
